We believe patients who stay closely connected to their own health information are better partners in their own care. It's part of why we built this practice the way we did - so you always have a clear line to your records and to us. We take that access seriously, and we're just as serious about keeping your information safe. A couple of the rules that guide how we handle your records are below.
HIPAA
The Health Insurance Portability and Accountability Act (HIPAA) is the federal law that keeps your health information private - it can't be shared without your knowledge or consent. You can read more directly from the source at HHS.gov, or in our full Notice of Privacy Practices.
Information Blocking (The 21st Century Cures Act)
The Cures Act is a federal rule that protects your right to see your own health information without unnecessary delay. In plain terms: your records belong to you, and we're not allowed to sit on them or make them hard to reach. The rule exists to keep patients engaged in their own care, keep care safer, and keep costs down. You can read more at HealthIT.gov.
How to Request Your Records
Log in to the patient portal to view your visit summaries anytime.
Submit a written request to our office for a full copy of your chart.
Records requests are typically processed within 5-7 business days.
Your Rights
Under HIPAA (the Health Insurance Portability and Accountability Act), your health information can't be disclosed without your consent or knowledge. Under the 21st Century Cures Act's Information Blocking rule, you're entitled to timely access to your own records.
